Understand support at home

Aging-in-place assessment

Understand what living well at home could take.

Changes in mobility, daily routines or confidence at home can leave families unsure what to do next. An aging-in-place assessment considers the person, their home and the support already around them.

Who this is for

For older adults, families and professional advisors who want to understand whether current living arrangements are meeting the person’s needs and what support to consider.

The support

What we help you understand.

01

Everyday life at home

Discuss the activities that are becoming more difficult, the person’s preferences and the help currently available. Their daily experience matters as much as the physical space.

02

The home and practical support

An occupational therapy perspective can help assess mobility, the home environment, equipment and support needs. Agree the assessment scope with ECI based on the concerns you want addressed.

03

A clearer next step

Discuss recommendations and whether help coordinating services would be useful. If a move is being considered, ECI can also explain its separate residence search service.

Getting started

A clear scope from the outset.

  1. 01

    Describe your concerns

Tell us what has changed, where the person lives and what you would like help understanding.

  2. 02

    Agree an assessment

The team confirms the appropriate professional, assessment scope, location, availability and fees.

  3. 03

    Consider the recommendations

Review the support to consider and agree whether further care management or residence-search guidance would help.

Before you decide

Your questions, answered.

Do we need to know what equipment or help is needed?

No. Explain the difficulties you are noticing. The purpose of the initial conversation is to identify the appropriate assessment and next step.

Does an assessment guarantee someone can stay at home?

No. Recommendations depend on the person’s needs and circumstances. The assessment helps inform choices about support and living arrangements.

Can you help arrange support afterwards?

ECI can discuss care management to coordinate the next steps. Confirm the scope and any additional fees separately.
