We’re Hiring: Intake Social Worker/ Geriatric Care Team Assistant

We are growing again! I am looking for an exceptional individual who is keen, motivated and eager to work with me in creating a difference in the lives of seniors and their families.

Requirements:

• BSW/MSW with minimum 3 years work experience with seniors
• Must be eligible or registered in the Ontario College of Social Workers
• Excellent command of English language- will be answering phones, providing information, ‘intake’ and writing reports
• Life or work experience with seniors and must have working knowledge of community resources for seniors
• Computer savvy in Word and must have excellent administration skills
• Able to work independently, is flexible and organized
• Able to drive, own car preferred, but not essential
• Understanding of case management model

Ideal candidate enjoys interacting with seniors and their families, and is able to multi task in a fast paced environment.

Responsibilities:
1) Admin, including but not limited to:

• Answering telephone and email inquiries
• Receiving client intake calls and assist with the coordination of referrals
• Preparing mailings and other documents as requested
• Assisting consultants with research regarding programs and services
• Setting up client files in both the electronic management system as well as hard copy
• Research and assisting with the writing of articles for publication
• Ensuring forms, directories, reports, research and other documents are current
• Sending faxes, emails and other documents as required

2) Case Management, including but not limited to:

• Comfortable in community setting- seeing clients in their own homes
• Assess needs of seniors and their families
• Work with Personal Support workers- providing support and direction
• Provide care management services as needed.
